Avaline Student Application Form

Please download and fully complete the Avaline Student Application Form. The form must be filled out in English. Once completed, the form should be signed by both the student and the guardian, then clearly scanned. Along with the application form, we also require the following documents to process your application:

• A copy of the student's passport or ID card
• The student's current school transcript and the graduation certificate for any completed courses
• If the student has studied in the UK or abroad, visa documentation must be provided
• If the student has completed an English language exam, please provide the results (IELTS, TOEFL, etc.)
• The student must submit the Avaline subject selection form
